These programs are cash only in our office. While we take a copy of your insurance card (if applicable) for urine testing, diagnostic services, and medication approvals, we do not bill your insurance company. Our staff is happy to provide a detailed statement for you to submit to your insurance company for reimbursement.

Completing New Patient Information prior to your appointment saves some of your time in the office. When handing in your paperwork at your appointment, please present the front desk with a photo identification and your insurance card. If you are a new Suboxone patient or a new Auto Injury/Workman's Compensation patient, please let our staff know at the time of making your appointment.
