To expedite new patient processing, please see the forms below for your first visit. A copy of valid photo identification and your insurance card (if applicable) is required at your first visit.

  • New Patient Packet  – Please print and complete these forms in their entirety. Be sure to bring them with you to your initial consultation.

 

Insurance

Pain Institute of Central Jersey and Neurology participates with most major insurance providers. Please see the list below for the insurance that we accept.

  • Aetna
  • Horizon Blue Cross Blue Shield – PPO plans exclusively
  • Cigna
  • GHI
  • Medicare – sorry, no Medicare at this time.
  • US Family
  • Qualcare

If you are unsure if your insurance provider is in our network, please call your insurance provider before making an appointment.

Patients with no insurance or an insurance provider not listed above are always welcome in our office as a self-pay patient. Please see our fees below for applicable costs.

 

At this time, insurance is not accepted for the Drug and Alcohol Addiction Program or for the Medical Marijuana program. Those programs are cash only. Please see our fees below for applicable costs.

Follow Up – $140
Appointment No Show – $50
Cancelled Appointment within 24 of Appointment Time – $50
Medical Records – $10 copy fee and $1 per page copied.
